Application to Renew Lapsed Membership

This form allows BMFA members to renew their membership. Members should fill out the application, enclose the appropriate membership fee, and mail the application to the BMFA Clerk. (The address is on the application.) 

Application to Become a New BMFA Member

This form allows people to apply for BMFA bronze membership. Applicants should fill out the application, enclose the appropriate membership fee, and mail the application to the BMFA Clerk. (The address is on the application.)

Bullard Farm Reservation Confirmation Form

After making a reservation with the Farm Caretaker, this form allows BMFA members to send information to the Farm Caretaker before visiting the Farm. 

BMFA Books and Pamphlets Order Form

This form is used by anyone wishing to purchase books and pamphlets from the BMFA.  

House Committee Feedback Form 

This form is used by BMFA members to provide feedback to the House Committee after a visit to the Bullard Farm. Members who stay at the Farm can provide feedback on the checkout form if they prefer, but this survey has more space to discuss any concerns.  

Paying Dues Form

This form is used by BMFA members to pay their dues. Fill out the form and send it with a check to the Treasurer. 

Volunteer Activity Documentation Form 

This form allows the BMFA leadership to identify and recognize volunteers who work to improve the Bullard Farm. Send a completed form to any Director, or the appropriate Committee Chairperson.  

Voting Proxy Document 

This document is used by BMFA members who can't attend an Association Meeting, and would like another BMFA member to vote for them.​
